Single-Site Membership Application
Thank you for your interest in joining our Association. We look forward to having you a part of our member community.
Our Site members are the healthcare facilities, senior dining communities, and systems that manage their own foodservice departments with individual operator contacts linked to the member Site.
The Site membership can be defined as a single hospital or senior dining location where at least one foodservice operation is self-operated and not contracted to a third party.
Annual dues are based on the number of contacts linked to each Site (location).
Membership term is based on a calendar year (January 1 - December 31). If you are joining after January, your dues will be prorated for the remainder of the year.
Instructions for Completing the Application:
- In the first step, you'll identify how many individuals you would like linked to your Site membership.
- The name and contact information provided in this application will be listed as the Site membership's primary contact. (If this needs to be adjusted afterwards, please email membership@healthcarefoodservice.org.)
- If you are signing up for a Site membership with more than one contact (yourself) linked, you will receive an email after your application is approved with instructions about how to add the other individuals.
- If you would like to pay your membership dues using Real Rewards Points, select to be invoiced for the dues and email membership@healthcarefoodservice.org.
