Thank you for your interest in joining and supporting our association. We look forward to having you a part of our member community. 

Business Partner Members are companies who supply goods and services to the healthcare foodservice industry. This membership includes one primary contact membership. Each additional contact you would like included with your company membership can be added for $230/contact/year. 


Membership term is based on a calendar year (January 1 - December 31).


Instructions for Completing the Application:

  • The primary contact linked to the company membership should include their contact information when filling out the "Personal Information" on this application.

  • This membership includes one individual membership linked to the organization. Each additional individual is $230/person/year. Indicate the total number of contacts, including yourself, you would like included with the company membership on the second page of the application in the "Information for Dues Calculation" section.

  • If you are signing up other individuals, in addition to yourself, to be included in your membership, you will receive an email after your membership has been approved with instructions on how to add the other individuals.
